Please complete the form below. The group leader and all participants must submit their forms at least one week before the booking date.

Anyone without a completed form will not be able to take part in the event.

October Half Term Party Consent Form

This form must be completed by all adult participants, and by the responsible adult for any participants aged 18 or under, as they will appear in Elysian’s social media and marketing materials.

Media Consent and Release

I give permission for photographs, video recordings, and/or audio recordings to be taken of me/my child during Elysian activities and events. I agree to the use of these materials for public display, advertising, and future promotional purposes, including:

Publicity Rights Waiver

By participating in this free event, I understand and agree to waive the right to deny use of photos, videos, or other recorded materials for Elysian publicity purposes now and in the future.
